Crown USA Inc. has been cited for eight serious and six other-than-serious violations of federal workplace safety regulations, the Occupational Safety and Health Administration announced Thursday.
OSHA investigators learned a maintenance technician was fatally crushed inside an unlocked hooding palletizer while servicing the machine. The company faces $98,699 in proposed penalties.
The company has 15 business days from receipt of its citations and penalties to comply, request an informal conference with OSHA, or contest the findings.
